Skilled Nursing Services at Home

For patients and families that choose to maintain skilled nursing services at home, they can be assured that the nurses, aides, and agency personnel that provide the help are acting in conjunction with the patient's primary physician.

While the doctor makes the directives, prescribes medication, and ultimately charges all care, it is the nurses and aides in the home that can serve as the first line for the patient. Through providing skilled nursing services at home, these professionals are in the position to hear the complaints of the patient, notice particular behaviors, or simply observe. In doing so, the nurse or aide acts as a liaison between the doctor and patient, potentially catching situations before they escalate.

The nurse or aide can help with communication in reverse as well. Patients and families often need doctor's orders and directives reiterated or defined in layman's terms, and their point of contact can be the visiting nurses. Visiting Nurses (In Valley Area)

Valley Home Health, visiting nurses in the valley area, firmly believes that home health workers play an instrumental role in facilitating the flow of communication between doctor and patient. They can help patients fully understand diagnoses, therapies, and set expectations. These nurses and aides also help the physicians by offering a third party, medically educated opinion on the patient's health status. Their regular presence in the home offers an unseen communication between the doctor and patient between visits.

To assist the visiting nurses and aides in this role is the Honeywell HomMed Monitoring System that is offered only by Valley Home Health. This technology gives daily information to the assigned nurse regarding his or her patient. The nurse can then note any changes or problems and get that information to the doctor as deemed necessary. Home care works best when communication flows easily between all parties, and this technology is a helpful tool in doing so.
