Licensed Clinical Social Workers

Assess the emotional, social, and environmental needs of the patients and utilize all available family and community resources to fasten patient adjustment and rehabilitation.

Qualifications:

• Must have a current/valid California SW license and CPR card.

RN, Field Clinical Area, Case Manager [Full-time or Part-time]

Qualifications:

• Must have a current/valid California Nursing license and CPR card.
• Minimum of 1-year experience as an RN in acute care setting.
• One-year experience in a home health agency is preferable.
• Able to exercise initiative and independent judgment.
• Excellent verbal and written communication skills.

Physical Therapists and PT Assistants; Occupational Therapists and OT Assistants; Speech Therapists [Full-time and Part-time]

Will provide homebound patients with services, which help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients suffering from injuries or disease.

• Current California PT License & CPR card.
• Minimum of 1-year experience in an acute care or rehabilitation setting; with preferred exposure to geriatric and medical/surgical patient care.

Licensed Vocational Nurse [Full-time and Part-time]

Communicate on a weekly and/or per needed basis with the Case Manager regarding patient’s progress and/or any changes.

• Must have a current/valid California LVN license and CPR card.
• Minimum of two (1) year experience as an LVN.
• Able to exercise initiative and independent judgment.
